Our adult day programs improve the quality of life for both participant and caregiver

Adult Day Program service is one of the many supportive services funded by the Central East Local Health Integration Networks. They are for individuals with chronic health conditions resulting in social isolation and inactive lifestyles. Day Program provides a safe, nurturing and stimulating environment. It also serves to provide relief to family caregivers.

The main objective is to allow the members to maintain constructive and healthy life routine despite their physical or cognitive conditions. All activities are well designed so as to build on their strengths and elicit feelings of success and well-being. Each member will have a case manager, who is a regulated health care professional, to develop an individualized care plan based on a thorough and comprehensive functional assessment.
