Senior Persons Living Connected (SPLC) contributes to the quality of life and healthy aging of older adults with a basket of services, healthcare and housing. Together, these support the complex social and health needs of older adults and their care partners. People access supports tailored to the individual including: interprofessional specialized geriatric and mental health and addictions services (GAIN Community Team, Adult Day Programs, Addictions Support Program), care management and co-ordination, in- home supports (personal care, homemaking, Meals on Wheels), exercise, health & wellness education.

SPLC is dedicated to supporting individuals impacted by the social determinants of health with access to proper care, and health literacy. Building on our 40+ year history of working collaboratively with our partners, we are an active partner in the Scarborough Ontario Health Team. We strive to deliver the best care and services to marginalized individuals such as newcomers, homeless / underhoused, and racialized communities.

We have strength in community outreach, community engagement, community mobilization, and provision of culturally competent programs. Our experienced and skilled staff and volunteers can communicate in multiple languages.
